Then the return is where to look. In these attached buildings the attic above the party wall is often continuous and the return duct up there leaks, so your system pulls from a space your neighbor's air also reaches. Cleaning changes nothing about that. Sealing the return box and the joints where it crosses the wall line generally ends it for good.

We own a duplex. Do both sides need doing?

For an inspection, yes, and this is the awkward part of a duplex. The two systems are separate but the attic they sit in is not, so a problem proven on one side is frequently produced by something on the other. Owners can arrange both easily enough. Where one side is occupied and access is refused we can still work, but the finding will be less certain and we will say so in the report.

Our 1990s house has never had the ducts done.

Then the flex is around thirty years old and the question is condition rather than dirt. We look at the strap points where insulation compresses, the bends where the liner tears, and the boots at the ceiling. If the runs are intact, cleaning is reasonable. If several sections have already failed, cleaning them is spending money on duct you will be replacing anyway.
