How often does the bulb need replacing?

We are in a manufactured home and the floors are cold no matter what we set the thermostat to.

Check the crossover before anything else. It is the flexible duct slung under the home between the two halves, and when it tears or drops off you are sending conditioned air into the crawl space. The symptom is exactly what you describe. This is inspected from underneath and repaired, and cleaning the floor registers would not have touched it.

There is a musty smell coming from the floor vents.

In a belly-duct system that often means moisture reached the trunk, either through a torn crossover, a gap in the belly board, or ground water under the home. We would rather find the water than treat the smell, because sanitizing a duct that keeps taking on moisture is a temporary purchase. Ground vapor control and closing the belly usually matter more than anything we can spray.

Our house was built in 2016. Do we need duct cleaning?

Very likely not, and we will tell you that before scheduling. Duct that age is normally in good condition unless there was construction work, a rodent problem or water. What is worth checking once is filter fit and whether every register is actually delivering, because a run kinked or crushed at framing during the build stays that way until somebody looks.
