The vents are noisy and the house never feels even.

Both are usually static pressure rather than dirt. We put gauges across the air handler and read what the system is actually doing. The common Watauga finding is equipment larger than the duct it got connected to, and the honest answer is then a return upgrade or a larger filter cabinet rather than a cleaning. That is less money than most people expect to be told.

There is one filter in the hallway ceiling and it is filthy inside a month.

A single undersized return does that. The filter face is too small for the air moving through it, so it loads fast and the pressure drop starts pulling air around the frame instead of through the media. Correcting the return area gives you a filter that lasts its rated life and actually filters, which is a better purchase than cleaning the system every year.

Ours is a small house. Is a full cleaning even worth it?

Often not, and the price should follow the system rather than a package. A modest single-story plan here has short runs, so once the plenum and the coil are clean there is not a great deal left to do.
